On-Line Database      

    You may view or edit your patient data using our On-Line SLN Database.  You must first register your institution and each physician that will be sending data.   Please use the Insitution and Physician Registration forms located on the Download Forms page.  A user name and password will be e-mailed to each physician who registers.  This will be sent to the e-mail address on the Physician Registration Form. 

    The user name and password are used with a small program that you must download from the appropriate link shown on the left of this screen.  We are in the process of building a multi-national database so please choose US Database if you are entering data for SLN Patients within the United States or China Database if you are entering data for SLN Patients within China.  This page will list forms you may use to send data and a link to download the access program.
