    These are on-line versions of the forms to send data to the China Sentinel Node Mapping Database.   Click on each form to download them to your computer.  Then complete the forms and fax back to 1-800-284-2190. 

    When the Institution and Physician have been registered a user name and password will be e-mailed to the account on the Physician Registration Form.  You will then be able to log on to the On-Line Database to view or enter your patient SLN data. 

    Download the following access program to your computer by clicking on the following link:  China SLN Connect.exe.  This program will allow you to enter your user name and password and then open the On-Line SLN Database.  Please note that you will only be able to see your own specific patient data and no others.
